Form B
The Form B Statement (the template for this statement can be downloaded from this page, below - Form B) is the statement of receipts and expenditure to be prepared by the Researcher, setting out, on an accrual basis, its annual receipts and expenditure for the Project, which must be in the form prescribed from time to time by the Corporation and available on this webpage.
The Form B statement of expenditure of the funds certified to be true and correct will need to be completed by a representative of your organisation who is authorised to sign financial statements.
A completed Form B should be forwarded to the GRDC at the following address:

An electronic copy is also required and must be emailed to forms@grdc.com.au.
If the Corporation does not receive both a satisfactory Final Report and the final Form B Statement within 3 months of the end of the Project Term, the Corporation will not be obliged to pay the Researcher the amount.
